We understand that filling out forms on your first visit can be daunting as you may not have answers to all the questions we ask. Although they can be seem tedious and unnecessary, the information that you provide through these forms will be used to accommodate any special needs / requirements prior to and during the treatments for the team.

To save you time from completing forms in our office, we have conveniently provided the new patient forms online for you to download, print, and complete prior to your arrival to our office. If this cannot be prefilled, that's ok too. In that case, please plan on arriving 15 mins prior to your scheduled appointment to fill out the necessary forms.
